When my hips go out of whack I actually don't get a whole lot of joint pain, but my butt, thigh, and lower back skin will get tender, almost like it is bruised. It will actually be painful to sit on it. So I would suggest that it could be caused by some sort of misalignment that he is not aware of. I go to a chiropractor who is a little more holistic, he doesn't pop any bones, he just shifts the muscles to persuade the bones back into alignment. I recommend this if you don't see someone already.
With that in mind, my grandmother has shingles and her first outbreak started with sore skin. Is it too late for him to try to get a vaccine? Her Dr. told her that shingles can be brought on by elevated stress, if he has been really stressed lately perhaps it may be shingles
Good luck finding a diagnosis, and best wishes for a speedy recovery
