Help with a survey

I have to pass out a survey to students at my high school about eating disorders. I have some ideas about what to ask (it's anonymous), but I figured people here might be willing to help out. What would you ask on a survey about eating disorders?

What is the goal of the survey? 'Cause my questions would vary depending on that.

Oops, I deleted that part. Basically, I'm just trying to find out how common eating disorders are at my HS.

I'd ask about what and how much people eat. If they pay attention to it or keep track or whatnot. If they count calories, fat, etc. What they think is an acceptable intake per day and why. I'd include some questions about body image. I would also ask questions about exercise and fitness, sleeping, coffee intake and intake of other psycho active substances (sugar, nicotine, whatever). Perhaps consult the DSM to include questions about symptoms.

You might ask how they feel after they have eaten. Do they visit any websites related to eating, weight loss, etc.

it would make for a better survey to include both undereating and overeating disorders.

^Yep, I've included that too. Some people don't realize that overeating IS a disorder.

If you google for an Eating disorders screening survey, that will give you what you need. They ask about body image, eating patterns, exercise, thought distortions, in addition to symptoms like bingeing, purging, restricting, food rituals, etc.

Ask about what sports they play...Dancers, Figuar Skaters, Cheerleaders and gymnasts are all pressured to have small, thin bodies. I have seen eating disorders first hand in gymnastics and they are more common then people would think
